WHAT SERVICES ARE AVAILABLE:

DO I QUALIFY FOR HELP:

Applicants must have a disability.

WHO DO I CONTACT:

Call for an appointment with intake counselor at the Main Offices in Boulder or Longmont. Service on a walk-in basis should not be expected. Clients can fill out Center paperwork during their intake appointment.

HOW DOES IT WORK:

The initial appointment is a detailed interview about the client's independent living barriers; they are encouraged to start the process of negotiating a plan of services that is most appropriate for them and in the order that is the most appropriate for them. The counselor can assist them with their plan if that is required.

WHAT ARE MY CHANCES FOR SUCCESS:

The client can expect to have a plan to improve their independent living status; this includes assistance with any issue barring self-sufficiency that the Center can contribute resources to meet this end. Often as not, this means help finding accessible housing, acquiring benefits, finding a job, and becoming skillful at living happily and comfortably in the community.

HOW MUCH DOES IT COST:

The Independent Living Program's full fee is $1500 per month, but is negotiated with the client, their families, their insurance companies, or Colorado Rehabilitation Services. Home Health Services are generally paid for by Medicaid/Medicare, but in a private pay situation, these are negotiated on a sliding fee basis. Employment Services are free to the client and are funded through grants and contracts with Colorado Rehabilitation Services and JTPA.
